Augment traditional primary care with wellness initiatives designed
to give Buckeye residents the knowledge and resources they need to
live healthy, fulfilling lives and manage chronic health issues.

Why is it important?

MetroHealth Buckeye Health Center's specially developed programs
in the following areas focus on conditions more prevalent in the community:

A measurable gap in health status existed

 
Primary care alone could not adequately
address the complex spectrum of health and
well-being needs in the Buckeye neighborhood

 

A focus on health BEFORE care was required

Taking on the challenge.
Closing the Gap programming includes: Asthma management Allergy management Diabetes self-management and education Freedom from Smoking® Senior wellness programming Weight loss and healthy lifestyle programming Children’s programming:
- Asthma Compliance and Technique (PACT) clinic
- NEW (Nutrition, Exercise and Wellness), a lifestyles program

with you for an hour and explain to you how you
should eat properly. I’m not going to be available
precisely at 4:00 p.m. when you’re ready to cook dinner...”

The MetroHealth System approached Saint Luke’s Foundation 
with a proposal for “Closing the Gap.” With a focus on developing community-based models to address the root causes of health disparities, this unique and unprecedented health and wellness
initiative was funded through a $10 million, 10-year grant.
