The Healthy People strategy focuses on two critical health concerns at both ends of the prevention and treatment continuum. On the prevention end, we seek to improve access to healthier food and engage in physical activity to reduce obesity. On the treatment end, we seek to promote a specific and proven strategy of care delivery we call the health home model of care.

We crossed the two-year mark of our strategies in October 2014, and we are very pleased with how we have defined ourselves within each specific priority. Moving forward, one primary goal on the obesity front is to look at ways in which we can support policy change. On the health home model of care front, one primary goal is to identify ways
to support providers beyond capacity for the creation of health homes.

For 2015, we have served  as a convener in both areas, enhancing learning and education as well as creating community conversation around both strategies. This has included our Cuyahoga County partners and grantees, as well as thought partners on state and/or national levels.
