Three Program Strategy Areas, One Mission
Urban Health and Well-Being
The Urban Health and Well-being strategy creates and catalyzes opportunities to improve community outcomes by addressing the root causes of disparities in health and healthcare. Initial priorities include behavioral, lifestyle and environmental issues; chronic disease prevention and management; and the development of a highly skilled, culturally competent healthcare workforce.
Urban Community Revitalization
The Urban Community Revitalization Strategy reflects the Foundation’s commitment to supporting the development of strong, vibrant urban neighborhoods of choice. With focus on Buckeye-Larchmere, Woodland Hills and Mt. Pleasant, this strategy addresses the social determinants that impact place, including neighborhood conditions, healthy eating/active living, food access, and land use. Meaningful engagement of community residents is integral to this strategy.
Urban Families
The Urban Families Strategy promotes family stability and success. Current priorities include housing, hunger and parent support with focus on families with children under the age of 18.

Our Commitment to Continuous Learning
We are excited to build upon our past investment in each of these three program strategy areas, and to identify
promising new models. As our program strategies evolve, we will further refine grantmaking in each area to ensure
our resources target opportunities with the greatest potential for impact. We will also remain open to addressing
pressing community needs as they evolve.
