Once a proposal is requested, please use the links below to download the proposal guidelines and application forms. Please contact Peg Butler if you have any questions or problems accessing the documents.

 

As a function of the partnership between the Foundation and the applicant organization, Saint Luke's Foundation staff members are available to offer support and guidance throughout proposal preparation. Organizations are highly encouraged to consult the Foundation with any questions that arise.

 

To avoid delays in moving your request forward in the decision process, please read our guidelines very carefully, download the appropriate forms, and use the checklist to ensure that all proposal requirements are met.

 

Proposals will not be considered if all required materials are not received in the appropriate format by the deadline date and time.

 

 

Proposal Guidelines and Application Form

 

The Saint Luke's Foundation wants to ensure applicant organizations have the tools necessary to complete a grant application to the foundation. To that end, please use the following documents to guide development of your proposal.

 

* Proposal Guidelines (MS Word)
* Checklist for Proposal Submission (MS Word)
* Logic Model (MS Word)
* Year-to-Date Final Logic Model (use only if this request is for continuing funding of a current grant (MS Word)
* Project Budget (Requested amount must be in whole dollars (MS Excel)
* Project Budget Narrative (MS Word)
* Operating Budget (Requested amount must be in whole dollars (MS Excel)
* Organization Revenue Form (MS Excel)

 

If your request is for general operating support, use the budget form labeled OPERATING BUDGET (do not use the PROJECT BUDGET form). Requests for general operating support do not require a Budget Narrative.
