SUBMITTING AN ONLINE PROPOSAL

In an effort to reduce paper and streamline our proposal process, the Foundation now has an online proposal form. If a full proposal is requested, an email will be sent to the organization’s contact person, with a link to the form. The form includes a link to all of the necessary documents (logic model and financial) shown below that can be downloaded, completed and saved. On the final page of the online form there will be directions to retrieve and upload these documents as well as board lists, audits, annual reports, etc. Those documents that might not be in an electronic format (audit, annual report) can be mailed or delivered to the Foundation by the proposal due date.
To avoid delays in moving your request forward in the decision process, please read all directions very carefully before beginning the online proposal form and downloading the required documents.
As a function of the partnership between the Foundation and the applicant organization, Saint Lukes Foundation staff members are available to offer support and guidance throughout proposal preparation. Organizations are highly encouraged to consult the Foundation with any questions that arise.
Proposals will not be considered if all required materials are not received in the appropriate format by the deadline date and time.

When you are working in the online form, there is a link that will take you to this page so that you can download the documents below. Save the documents on your computer, complete the appropriate information and upload them prior to submitting your proposal.
Only the audit, annual report and support letters may be mailed or delivered as hard copies if they are not available electronically or cannot be converted to electronic documents.

* Project Budget (requested amount must be in whole dollars) (MS Excel)
* Operating Budget (requested amount must be in whole dollars) (MS Excel)
* Organizational Revenue Form (MS Excel)
*Project Budget Narrative (MS Word)
If your request is for general operating support, use the budget form labeled OPERATING BUDGET (do not use the PROJECT BUDGET form). Requests for general operating support do not require a Budget Narrative.
